Student Participation Agreement and Waiver

This is a legally binding agreement affecting important rights. Read each acknowledgment before checking it. By signing, you confirm you had an opportunity to ask questions and consult an attorney.

1. Voluntary Participation and Assumption of Risk

I understand that fire-service training is inherently hazardous and can cause property damage, serious injury, permanent disability, paralysis, or death. I participate voluntarily, will follow all safety directions, and accept all known and unknown risks, including risks arising from the ordinary negligence of a released party, to the fullest extent permitted by law.

2. Release of Liability and Covenant Not to Sue

To the fullest extent permitted by Texas law, I release CTRTA and the other released parties from claims arising from training, including claims based on ordinary negligence, and agree not to sue for released claims. This does not release gross negligence, reckless or intentional misconduct, fraud, or rights that cannot legally be waived.

3. Personal Property, CTRTA Property, Facilities, and Equipment

I am responsible for my property, proper care and return of assigned equipment, reasonable repair or replacement costs caused by my negligent, reckless, intentional, dishonest, or unauthorized conduct, and the limited indemnity terms stated in the agreement.

4. Medical Readiness, Emergency Response, and Insurance

I am responsible for determining my medical readiness, reporting symptoms or injuries, and paying medical expenses not covered by insurance. If I cannot provide informed direction, I authorize reasonable first aid, removal from an evolution, and activation of emergency medical services.

5. Photo, Video, Audio, Name, and Likeness Release

Select one. Safety, identity, skills, incident-review, and certification documentation may still be created and retained even if promotional consent is declined.

6. Testing, Skills Evaluation, Certification, and Course Completion

I will follow CTRTA, proctor, facility, TCFP, SFFMA, college, and course-specific testing and skills requirements. Enrollment, completion, remediation, or retesting does not guarantee a passing result, certification, employment, or licensure.

7. Academic Integrity and Test Security

I understand what constitutes cheating, plagiarism, falsification, unauthorized use of AI or answer sources, improper retaking, and sharing restricted test content. I will protect test security and accept that violations may result in invalidated work, failure, dismissal, enrollment restrictions, or referral to an authority.

8. Safety Rules, Conduct, Removal, and Dismissal

I will follow CTRTA and host-site rules and lawful instructions. Unsafe, dishonest, disruptive, impaired, threatening, harassing, or damaging conduct may result in restricted participation, removal, remediation, or dismissal without a guaranteed make-up, refund, passing status, or certification.

9. Legal Terms and Final Confirmation

Texas law governs this agreement; electronic copies and signatures may be treated as originals; invalid provisions may be modified or severed; and this agreement supplements CTRTA policies. I have read the entire agreement, had an opportunity to ask questions and consult counsel, and sign voluntarily.
